Insurance Requirements
Worker’s Compensation is required in the state of Oklahoma (this is not required on our Texas properties). A certificate with valid dates must be submitted before we will consider any firm. You will need one of the following:
  • If you are a family owned and operated business, each family member must sign a disclosure statement on each invoice for which you receive payment stating (in summary) that you are a family of five (5) or fewer and you will waive your rights to worker’s compensation insurance.
  • Liability Insurance of $500,000 or more, depending on the work being performed.
*Please Note: All payments will be made to the exact name listed on all insurance certificates.
Other Requirements
All current and prospective Independent Contractors must have a W-9 on file with DS Property Network. Click here to download a copy.
A signed DS Vendor Agreement. These Agreements are available for review at the time of bidding or of submission of documents on this page.
Unconditional Lien Waivers – signed by the appropriate members of your company when the work is complete.
NOTICE:
No payment will be made if DS Property Network does not have current valid insurance certificates or any of the documentation described above.
